To secure a card, prepare two copies of the PMRF, two recent 1x1 photos, and at least two valid government IDs. You can submit these files to any PhilHealth Local Health Insurance Office (LHIO) or branch in your city.
Step-by-Step ID Application
- Visit your nearest PhilHealth Local Health Insurance Office (LHIO).
- Get a queue number and fill out two copies of the PMRF.
