Why is the Member Data Record (MDR) Required?

Hospitals use the MDR to verify that your dependents are registered. If a dependent child or spouse is hospitalized and their name is not printed on your MDR, the hospital billing section will reject the automatic benefit deduction, forcing you to pay full rates and file a manual reimbursement claim.

Download Procedure Steps

  1. Go to the PhilHealth Portal homepage (https://memberinquiry.philhealth.gov.ph/member/).
  2. Enter your PhilHealth Identification Number (PIN) and password. Resolve the security captcha.
  3. Click Login. If logging in for the first time, click 'Register' to create credentials.
  4. On the user dashboard home page, locate the sidebar menu.
  5. Click on 'Member Information' or 'Print MDR'.
  6. Your current MDR will load as a PDF document on screen. Click the 'Download' icon to save it, or the 'Print' icon to print directly.

Verify Your Spelling

If you notice errors in your name spelling, birthday, or dependent list on the downloaded MDR, do not try to edit the PDF yourself. You must submit a PMRF with the 'Correction' checkbox ticked to a local PhilHealth branch along with supporting PSA certificates.
